Overview We are seeking a dynamic and experienced Homecare Clinical Manager - Registered Nurse to lead our clinical team. This vital role involves overseeing patient care delivery, managing nursing staff, ensuring compliance with healthcare standards, and promoting excellent patient outcomes. Join us to make a meaningful difference in the lives of those we serve while advancing your nursing leadership career. Duties Receives and coordinates case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Supervise Intake to make sure appropriate clinicians are assigned to case. Facilitates, coordinates and integrates orders from all physicians. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services. Available during operating hours to assist clinicians as appropriate. Reviews and critiques all clinical staff documentation. Assures accuracy and appropriateness. Performs QA & review coding. Attends and facilitates case conference meetings with agency personnel to facilitate coordination of care. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate agency personnel. Assists in the screening and interviewing process of new agency personnel and makes recommendations for employment of individuals. Assists in the orientation of new agency personnel. Assists Clinical Director in the planning, implementation and evaluation of in-service and continuing education programs. Participates in agency Quality Assessment and Performance Improvement Projects as assigned. Serves as a backup support for Director of Clinical Services. Skills Registered nurse with current licensure to practice nursing in State of Michigan. BSN preferred. Previous experience in home care setting preferred. Proven ability to work in an interdisciplinary setting. Proficiency in medical terminology, physiology knowledge, and diagnostic evaluation techniques. Strong leadership skills with supervising experience in a healthcare setting. Knowledge of homecare services including dementia care, assisted living, long-term care, and behavioral health. Understanding of
ICD-9/ICD-10
coding standards for accurate medical documentation. Knowledge of infection control protocols and aseptic techniques to ensure patient safety.
