Job Description
Wellness Coordinator Make a Difference Every Day Turning Leaf is seeking a compassionate, organized, and motivated Wellness Coordinator to oversee and coordinate healthcare services for adults receiving residential support. This leadership role works closely with physicians, psychiatrists, pharmacies, guardians, and our care teams to ensure residents receive exceptional medical and behavioral health support. What You'll Do ✔ Coordinate medical, psychiatric, dental, vision, and specialty appointments ✔ Manage medication-related communication with providers and pharmacies ✔ Maintain health records and ensure regulatory compliance ✔ Monitor resident wellness, weights, labs, and follow-up care ✔ Train and support Direct Support Professionals on medication administration and wellness practices ✔ Participate in admissions, transitions, and clinical team meetings ✔ Serve as a key liaison between healthcare providers, families, and the Turning Leaf team Qualifications ✔ 2+ years of experience working with SPMI, IDD, Adult Foster Care, or related populations ✔ Strong organizational and communication skills ✔ Management or supervisory experience preferred ✔ Valid driver's license and acceptable driving record ✔ Ability to complete required trainings including CPR/First Aid, Recipient Rights, Medication Administration, and MANDT What We Offer ✅ Competitive salary ✅ Health, dental, and vision benefits ✅ Paid time off ✅ Professional development and training opportunities ✅ Meaningful work with a supportive team environment Why Turning Leaf?
