Full-time Job Overview:
TowerLight Senior Living is located in historic St. Louis Park, Minnesota. Only 10 minutes from downtown Minneapolis, we are near public transportation and the crossroads of Highways 7 and 100. Our community offers 115 units of beautifully appointed Independent Living Apartments, innovative Assisted Living Apartments, state-of-the art Memory Care and Enhanced Care Suites (as well as an intergenerational day care). Our tenured team ensures a strong commitment to delivering high end comfort and the best senior care available for our residents. Part of Ebenezer, Minnesota's largest senior living operator whom has been providing excellent care for seniors for over 107 years. The Assistant Director of Health Services Nurse is responsible for ensuring that the safety and health needs of the senior assisted living, memory care and enhanced care suite patients are met while providing staff leadership duties. Role is supported by a DHS RN, resident assitants/med passers, regional nurse, triage nurses. This RN schedule includes; 80 hours every two weeks ; Full Time 8:00am - 4:30pm, Days Full benefits package offered, including RN student loan repayment option (up to $2,000 per year over 5 consecutive years with work commitment) Nursing Responsibilities Maintains current assessment and documentation of client's health status. Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition. Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement. Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. Maintains clinical charts and progress notes using computerized documentation system. Communicates changes in client's conditions to medical and other relevant individuals. Coordinates client health services. Facilitates coordination of services with internal and external providers. Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations. Works with client and/or representative(s) to assure delivery of appropriate services and funding. Participates in Care Conferences as needed or requested by the DHS or Executive Director Required Qualifications Vocational School /Technical Training or Associates Degree in Nursing One to Three Years Exerpience MN Registered Nurse (RN) License Basic Life Support (American Heart Assoc or Red Cross) Preferred Qualifications Bachelor of Science Nursing Three to Five Years Experience Prior supervisory experience Prior experience in home care or assisted living environment
