Licensed Practical Nurse - Charge Nurse

Responsibilities Lead and supervise nursing staff to deliver exceptional patient care across various departments, including long-term care, acute care, and assisted living facilities. Coordinate daily clinical operations, ensuring efficient workflow and adherence to safety protocols. Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform treatment plans. Administer medications, injections, IV infusions, and perform catheter care following strict medical documentation standards. Oversee medical procedures. Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to optimize health outcomes. Maintain accurate medical records using EMR (Electronic Medical Records) systems while ensuring HIPAA compliance. Assist in diagnostic evaluations including medical imaging and laboratory specimen collection/processing. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, infectious diseases, or those requiring hospice or palliative care. Skills Proven experience with Epic EMR/EHR systems. Extensive critical care experience Strong background in geriatrics, memory care (including Alzheimer's and dementia) Skilled in IV insertion/infusion therapy, phlebotomy, medication administration, and aseptic techniques. Knowledge of anatomy and physiology to perform thorough patient assessments and diagnostic evaluations effectively. Familiarity with medical terminology related to internal medicine, surgical procedures, pain management, and specialty areas like ophthalmology or cardiology. Excellent communication skills Capable of managing urgent situations such as acute pain episodes or infectious disease outbreaks while maintaining composure under pressure.
