  • BH•FT (Mon-Fri, 8:00am•4:30p) 101 Truman Medical Center Job LocationUH Behavioral Health Kansas City, Missouri Department Community Mental Health Center Position Type Full time Work Schedule 8:00AM•4:30PM Hours Per Week 40 Job Description The Nurse Care Manager (NCM) is responsible for lateral integration of care across all settings in which health care is delivered to his/her assigned patients.
Through the assimilation and application of evidence-based information, the NCM designs, implements, and evaluates client treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working with the health care system to promote an environment that will empower his/her assigned patients to manage their illness(es) and achieve optimal outcomes.

The NCM will provide and manage care of assigned community clients. Minimum Requirements BSN or the equivalent in a combination of an ADN and relevant experience Current Missouri License as a Registered Nurse Minimum of three (3) years experience working in a healthcare setting Excellent written and verbal communication skills Must be able to read and speak the English language Proficient in Word, Excel and Power Point Current BCLS Driving is a requirement for the employee. He/she must meet the requirements specified in the Fleet Driving Safety and Compliance Policy and must continue to meet these requirements as a condition of employment
Preferred Requirements:
Behavioral health/mental health service experience Community health experience
