February 23, 2024 Maryville Addiction Treatment Center has a staff of dedicated professionals who devote themselves each day to helping people suffering from an addictive disease reclaim their lives. Our staff provides comprehensive high quality substance abuse and co-occurring treatment and recovery services to individuals and families affected by the disease of addiction. Basic Function:
A Registered Nurse Care Coordinator is responsible for active care coordinator of clients receiving appropriate care, screensing and referring client to other providers as needed. Provides administration of medications and record this activity in the client's medical chart. Ensure adequate medical emergency supplies, as well as securing necessary medical records on all clients.
Essential Functions:
1. Assures that all clients receive active care coordination in receiving the care, this includes but is not limited to: making appointments for clients, answer questions or concerns that a client may have about a referral, following up with a referral to assure that the client attends, addressing any barriers to client attendance. 2. Develops protocols for physical health screening and assessment. 3. Screens, or reviews of screening completed by others, of 100% of all clients for hepatitis C, HIV and other related health conditions upon admission and at regular intervals. 4. Provides or refers clients to other practitioners, such as an FQHCs for vaccinations and medications for Hepatitis A or B and HIV when appropriate. 5. Coordinates with prescribers at all community medical providers. 6. Provides delivery and/or review of the client nursing assessment. 7. Monitors health risks by providing screening, preventative care, analyzing lab/screening reports, and referring to treatment, where needed. 8. Develops or contributes to the medical components of the client care plan. 9. Monitors medications and medical treatments for potentially adverse interactions and effects, developing strategies to reduce or eliminate polypharmacy, and intervening where needed to protect the health and well

10. Develops and periodically revises medical aspects of the care plan based on information collected through client assessments, review of records, consultation with any treating physician and input from client and family. 11. Interfaces with Medicaid and private insurers to assure that information needed for medical coverage is available to the payer. 12. Investigates and whenever possible, obtains services for clients who are uninsured. 13. Ensures the provision or coordination of high-quality health care services that are informed by evidence-based practices and coordinating preventative and health promotion services. 14. Interfaces with specialty medical services (e.g., cardiology, hepatology, nephrology, endocrinology, infectious disease) to include, but not limited to, referral to specialty practice, facilitation of the referral, communicating medical needs identified at the OTP, collecting records from the specialty care service provider necessary to inform care at the OTP, sharing records from the OTP to specialty care provider, periodically reviewing progress at the specialty care provider. 15. Performs all other duties as assigned by the Assistant Director of Nursing or Director of Nursing. Required Knowledge, Skills and Abilities
  • Knowledge of health care issues pertaining to substance use disorders is required, including but not limited to mental health needs, women's health issues, HIV/AIDs, and other communicable diseases.
  • Knowledge of accurate record keeping in accordance with regulatory agencies and standards is required.
  • Knowledge of nutritional requirements and practices is required.
  • Knowledge of therapeutic communication techniques is required.
  • Must be able to work with clients to gain confidence and cooperation while maintaining boundaries.
  • Excellent prioritization and time management skills is required.
  • Excellent organizational and detail-oriented skills are needed to maintain accurate records and reports.
  • Must have the ability to establish and maintain effective working relationships with clients, staff, health care providers, outside agencies, institutions and the general public.
  • Knowledge of Microsoft Office programs (Word, Outlook) is required.
Minimum Requirements Education:
Graduate of an accredited School of Professional Nursing is required. Current RN license in good standing in New Jersey is required. Current First Aid/CPR Certification issued by the American Red Cross or the American Red Cross Association required.
Experience:
May be recent graduate of approved nursing school or a minimum of one (1) year of nursing experience.
Sobriety:
If recovering, must have at least two (2) years of continuous sobriety and maintain sobriety throughout employment.
