Job Description

Overview Join our compassionate team as a Hospice Clinical Manager RN, where you will lead and coordinate high-quality end-of-life care for patients and their families. This dynamic role offers the opportunity to oversee clinical operations, mentor nursing staff, and ensure compliance with healthcare standards. As a vital part of our organization, you will drive excellence in patient care delivery while fostering a supportive and collaborative environment. Your expertise in critical care, medical documentation, and team management will be instrumental in providing dignified and personalized hospice services. Responsibilities Supervise and manage nursing staff, including scheduling, performance evaluations, and ongoing training to maintain high standards of patient care. Oversee patient assessments, ensuring comprehensive evaluations of physical, emotional, and psychological needs using advanced physiology knowledge and medical terminology. Coordinate interdisciplinary team efforts to develop personalized care plans aligned with patient wishes, including symptom management such as acute pain control and dementia care. Ensure accurate medical documentation utilizing EMR (Electronic Medical Records) systems like Epic or Cerner, adhering to HIPAA regulations and NCQA standards. Monitor clinical compliance with healthcare laws and regulations related to hospice care, including discharge planning, medication administration, and infection control protocols. Facilitate communication with patients' families and caregivers to provide education on disease progression, nutrition, tube feeding, and end-of-life decisions. Lead utilization review and management processes to optimize resource use while maintaining quality standards in hospice services. Experience Proven leadership experience supervising nursing teams within a hospice or palliative care setting. Extensive critical care experience in settings such as ICU or Level I Trauma Centers, with familiarity in acute pain management and ventilator support. Strong background in medical-surgical nursing, geriatrics, behavioral health, or long-term care environments including assisted living or nursing homes. Hands-on experience with EMR/EHR systems like Epic, Cerner or Athenahealth for accurate documentation and patient record management. Knowledge of
ICD-9/ICD-10
coding for accurate medical billing and documentation purposes. Experience working with diverse populations including individuals with developmental disabilities or dementia; familiarity with working with children or neonatal ventilator care is a plus. Demonstrated ability to manage teams effectively while maintaining compliance with healthcare standards such as NCQA or OSHA regulations.
