Position Overview:
Seeking an experienced Registered Nurse (RN)
  • Care Manager
  • ED with Care Coordination, Utilization review .
Key Responsibilities Coordinate patient care and discharge planning activities from the Emergency Department. Conduct utilization review to ensure appropriate level of care and resource utilization. Assess patient needs and develop individualized care transition plans. Collaborate with interdisciplinary teams to facilitate timely and safe patient discharges. Connect patients and families with community resources and post-acute services. Maintain accurate documentation and ensure compliance with regulatory requirements. Participate in care coordination initiatives to improve patient outcomes and reduce readmissions. Utilize electronic health records and care management systems effectively Education Associate Degree in Nursing (ADN/ASN) from an accredited nursing program. Bachelor's Degree in Nursing (BSN) preferred. Experience Minimum five (5) years of clinical nursing experience.. Care Coordination Utilization Review Discharge Planning Licensure & Certifications Active Registered Nurse (RN) license in the State of New York. PRI and Screen Certification (must obtain within three months of employment). American Heart Association (AHA) Basic Life Support (BLS) certification.
