Overview:
Join a dynamic team at EEH as a Clinical Nurse Manager leading an interdisciplinary team of nurses, social workers, home health aides, and chaplains by motivating them to provide excellent, quality, patient-family centered care. The Clinical Nurse Manager has the responsibility and authority for planning, coordinating, implementing, and evaluating patient care services and staff education related to hospice care. Responsible for managing the day-to-day clinical operations of the assigned interdisciplinary team.
What you will do:
Ensure adequate safe staffing with the Assistant Clinical Manager and Staffing Administrator. Complete timely annual performance appraisals. Participate in interviewing, hiring, and orientation of staff members. Triage and respond to patients and families' questions related to the care plan. Coordinate and lead the weekly Interdisciplinary Team meeting. Collaborate with the intake department to facilitate timely enrollment. Monitor the Staff Administrator staff schedule to maintain safe staffing levels. Analyze staff documentation for compliance. Demonstrate responsiveness to patient/family and staff concerns. Pursue continuing hospice education to maintain current knowledge with best practices. Competency to make patient visits when necessary. Participate in implementation of QAPI initiatives. Identify trends in departures of staff clinical practice. Report patient/family/staff complaints to DON or VP. Function as a resource for clinical, finance and administrative staff. Participate in supervisor on call rotation, weekdays, holidays, and weekends. What you will bring to the position: Extensive knowledge of hospice philosophy and the COPs. Proven leadership experience. Support EEH's mission and vision. Promote a just culture and a respect for a culturally diverse workforce and patient population. Exceptional organization skills, emotional intelligence, flexibility, the talent to prioritize. Lead by example; demonstrating integrity, respect, commitment, empathy and transparency. Experience in end-of-life symptom management. Excellent people skills to motivate the IDT with team building. Effective in both verbal and written communication skills and critical thinking. Familiar with the NYSDOH hospice regulations.
Qualifications:
A NYS registered professional nurse license. BSN Required, Masters preferred. CHPN, preferred. Mandatory to obtain within 2 years from date of hire. A minimum of 4 years hospice experience.
