Creative Foundations, a Help at Home Company is the nation's largest provider of home-based care, serving individuals across multiple states and service lines including unskilled home care, Intellectual and Developmental Disabilities (IDD), Structured Family Care, and Home Health. The Registered Nurse — Clinical Services Supervisor serves as the primary clinical authority for the Creative Foundations program in Ohio, operating at the intersection of DODD compliance, clinical quality, and workforce readiness. This role is accountable for medication administration oversight, delegated nursing functions, DSP clinical competency, DODD and ODA regulatory compliance, and quality review. The RN serves as the designated Medical Director for CLIA waiver purposes and provides direct clinical governance across all Ohio IDD service sites. This is a high-accountability, high-visibility position requiring independent clinical judgment and regulatory fluency.
Key Responsibilities:
1.Medication Administration & Clinical Oversight Conduct monthly MAR audits covering 100% sample of PRN orders for completeness, accuracy, indication, dose, frequency, route, and max dose; track and resolve discrepancies within 48 hours Perform full quarterly MAR reviews for all residential and ADS individuals; oversee MAR off-compliance rate weekly Serve as clinical liaison between SLCs and physician offices for PRN order corrections; provide standardized order correction letter templates; escalate unresolved orders within 48 hours Maintain and annually review all medication policy documents including PRN Order Standards, Controlled Substance Protocol, Biohazard Waste Disposal, Standard Precautions, and Glucometer/CLIA Policy 2.Medication Certification & Delegation Training Deliver in-house DODD Medication Certification (Cart 1, 2, and 3) classes for all new hires and annual recertification; maintain DODD-approved curriculum Perform all running delegation for Cart 2 (GJ tube) and Cart 3 (insulin) individuals in both residential and ADS settings; maintain full re-delegation tracking for all currently delegated individuals Conduct annual per-individual skills checklist verification at each delegation visit for all delegated individuals including Home Lifts, Gait Belt, Wheelchair/Adaptive Equipment, Glucometer Operation, GJ Tube Care, and Insulin Administration Replace current dependence on Dynamic Pathways for all medication delegation functions; no external vendor backup 3.CLIA Waiver & Medical Director Functions Serve as designated Medical Director for the CF CLIA waiver, overseeing glucometer/sliding scale testing across all Ohio sites Interpret sliding scale results and ensure DSPs implement within documented parameters; maintain CLIA waiver application pending licensing and site engagement Ensure lab test interpretation aligns with CLIA regulations under 42 CFR Part 493 4.Program Management —

Act as primary Nurse Manager for DODD and Ohio Department of Aging (ODA/Passport) programs Oversee development, implementation, and currency of Individual Service Plans (ISPs) and Person-Centered Care Plans Ensure all home health services meet state auditing standards; maintain survey-ready documentation across all Ohio IDD counties Coordinate with county boards, COGs, Judy Dalton, Tarrytown Pharmacy, and DODD regional staff 5.Internal Quality Reporting & Review Cycle Produce monthly clinical quality report covering med error rate by SLC territory, PRN order completeness, EMAR compliance percentage, delegation currency status, incidents/MUI trend, and medication-related events Present findings in monthly quality review meeting; flag chronic sites for targeted intervention and corrective action tracking Maintain zero internal clinical quality reporting gaps; eliminate the current state of no internal med error rate data 6.Skills Checklist Development & Competency Verification Create and maintain clinical skills checklists for DSPs covering all high-risk interventions; validate competency annually per individual Ensure skills checklists are confirmed for new hire onboarding; document in personnel files via Advisor Conduct or supervise competency verification for all delegated DSPs in alignment with
