Summary:
In accordance with the mission and values of The Village of St. Edward and part of the management team of the Skilled Nursing Department, the Clinical Supervisor/Staff Educator of LTC/SNF, plans, develops, organizes and conducts education and onboarding programs and training primarily for clinical staff in the skilled nursing department as well as provides education to families and residents regarding their care. In addition, this role provides direction and supervision of clinical function; monitors staffing; on- call rotation and performs other job-related duties as assigned.
Qualifications:
Current license to practice as a Licensed Practical Nurse (LPN) in the State of Ohio At least two (2) years of experience in a supervisory type of position At least three (3) years of experience in an organization providing services to the elderly. At least two (2) years of experience of providing training/education to staff Current BLS certification Participates in nurse on-call rotation with nurse management team Good physical and mental health. Possesses the necessary dexterity to perform injections, IVS, etc. Able to lift, push and pull 35-50 pounds independently, and over 50 pounds with assistance. Possesses the ability to bend, stoop, and reach at different angles. Walks, stands, and sits intermittently during the day. Must work effectively with residents, families and public on personal basis. Demonstrates effective communication skills in order to speak/educate with families, residents, and staff Ability to manage and supervise nursing staff. Demonstrates evidence of leadership abilities, clinical competence and interpersonal skills. Knowledgeable of principles of growth and development for the entire life span with the ability to provide care appropriate for the adult and geriatric residents. Job Duties (not an all-inclusive list): Plans, develops, organizes, and conducts on-site, hands-on training to staff Develops programs, materials and events to teach staff/residents and family about health topics Plans, develops, organizes, and conducts onboarding training for new staff as needed Educates staff in use of the electronic medical record/PCC and other systems as needed May assist with educational information for and during new hire orientation Audits Relias training of staff and compliance Serves as an educational resource to staff Assists with mask fit testing Conducts audits of documentation compliance and follow-up. Maintains documentation of training. Completes hands-on clinical audits and follows up with
DON/ADON
as needed. Assists with family and resident discharge education, medication, notification and documentation. Develops, educates and assists staff in performing new/revised policies and procedures as needed. Participates in performance improvement activities including survey preparation as requested. Provides regular feedback on individual employee's performance. Assumes staff nurse responsibilities as needed. Assists with staff interviews in absence of ADON and DON Assists DON and ADON with 90 day and annual evaluations of staff Demonstrates techniques for providing open, professional communications with residents, families, physicians and staff. Recognizes and acts upon resident, family and team concerns in a timely manner and reports results of the problem resolution to the appropriate person(s). Develops and maintains collaborative relations with members of the interdisciplinary team, other departments and outside agencies. Follow-up appointments- audit and notify residents and family of appointments and any changes Maintains accountability for the care of residents in areas of responsibility. Shares on-call responsibility, including rotation of weekends/holidays Demonstrates critical thinking skills. Assists to develop leadership and decision-making skills among nursing staff members. Uses the problem-solving process appropriately, either individually or in collaboration with others. Supervises and provides direction to nursing staff. Delegates tasks effectively. Educates staff in use of the electronic medical record; serves as a resource to staff. Makes daily rounds on each floor. Evaluates nursing care provided. Recognizes staff noncompliance with policies and procedures; provides instruction for staff as to correct policy/procedure; assesses the need for and initiates progressive discipline.
