Enliven Home Health Position Summary:
Enliven Home Health is seeking an experienced and compassionate Clinical Manager, Registered Nurse (RN) to provide leadership and oversight of our clinical operations. The Clinical Manager is responsible for ensuring the delivery of high-quality, patient-centered home health services while maintaining compliance with state, federal, and accreditation standards. This role serves as a key leader for clinical staff, supporting care coordination, quality improvement initiatives, regulatory compliance, and the achievement of positive patient outcomes. The ideal candidate combines strong clinical expertise with exceptional leadership skills and a passion for serving patients in the home setting. Key Responsibilities Clinical Leadership & Operations Provide day-to-day oversight of clinical operations and patient care services. Supervise and support nursing and interdisciplinary clinical staff, ensuring adherence to agency policies and standards of care. Conduct clinical record reviews to ensure accuracy, completeness, and regulatory compliance. Monitor patient outcomes and implement quality improvement initiatives to enhance care delivery. Participate in patient care conferences and collaborate with physicians, therapists, and other healthcare professionals. Regulatory Compliance & Quality Assurance Ensure compliance with Medicare Conditions of Participation, state regulations, accreditation standards, HIPAA requirements, and agency policies. Oversee quality assurance and performance improvement (QAPI) activities. Monitor documentation practices within the EMR system to ensure timely and accurate completion. Assist with survey preparedness and corrective action plans as needed. Staff Development & Supervision Recruit, onboard, train, and mentor clinical staff. Conduct competency assessments and performance evaluations. Provide coaching and clinical guidance to support professional development. Promote a culture of teamwork, accountability, and excellence in patient care. Care Coordination Support comprehensive patient assessments and individualized care planning. Collaborate with referral sources, physicians, hospitals, and community partners to ensure seamless transitions of care. Oversee case management activities, including admissions, recertifications, transfers, and discharges. Ensure appropriate utilization of resources while maintaining exceptional patient outcomes. Qualifications Required Current and unrestricted Registered Nurse (RN) license. Minimum of three (3) years of clinical nursing experience. Minimum of one (1) year of supervisory or management experience in home health, hospice, or healthcare operations. Knowledge of Medicare regulations, home health documentation requirements, and clinical best practices. Experience with Electronic Medical Records (EMR) systems. Strong leadership, organizational, and communication skills. Current Basic Life Support (BLS) certification. Preferred Home Health management experience. OASIS assessment and documentation expertise. Knowledge of Kantime or similar home health software, including Epic and SHP. Experience with quality improvement programs and regulatory surveys. Additional certifications related to home health leadership or case management.
