Minimum Qualifications:

RN license required Leadership Tactful Knowledge of nursing and medical practices and procedures Care Manager experience is preferred

Job Duties:

Facilitate patient care by assessing enhanced care - long-term behavioral health patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress. Facilitate and actively participate in interdisciplinary planning of clinical care of patients and assist with outcome report completion. Serve as care liaison between patients, physicians, care team members, and discharge planner by coordinating, monitoring, and communicating patients' progress. Initiate and implement care plans as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress. Communicate discharge information to physicians and payers with outcome data, as appropriate. Make written and oral reports/recommendations concerning the activities of your shift as required. Ensure that all nursing service personnel comply with their respective job descriptions. Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing service department. Participate in facility surveys (inspections) made by authorized government agencies as may be requested. Conduct accident/incident investigations and give completed reports to the Director of Nursing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting Join a team leading change in the industry! Mt.
