As a key member of our wound care team, you will: ➢ Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities ➢ Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities ➢ Lead the charge on wound assessment, treatment planning, and advanced procedures ➢ Harness cutting-edge tech and evidence-based protocols to accelerate healing ➢ Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families ➢ Keep your charting sharp and timely in our customized EMR ➢ Help drive quality initiatives and support our mission of raising the bar in post-acute care

Qualifications:

Active Pennsylvania Nurse Practitioner license (board-eligible not accepted) or Physician Assistant Prescriptive Authority and DEA license•must obtain by end of training period At least 2 years of wound care experience Previous experience in SNFs, long-term care, or rehabilitation facilities preferred Willingness to travel to designated partner facilities in several nearby counties Comfortable with EMR, mobile, and laptop documentation Strong interpersonal skills — collaborative, communicative, and adaptable As we grow, you'll be a part of a supportive, forward-moving company that empowers Providers to do what is best for their patients, every time.
