GENERAL DESCRIPTION
ALS Network exists to support people living with amyotrophic lateral sclerosis (ALS) and their loved ones through compassionate care, meaningful connection, and trusted resources. We partner with the ALS community to improve quality of life today while advancing advocacy efforts that elevate urgent needs, influence policy, and expand access to care, while driving the discovery of prevention strategies, treatments, and cures for tomorrow. Our work is rooted in the belief that no one should face ALS alone. The Part Time Care Manager for The Big Island will serve ALS patients and families in a variety of areas including psychosocial support, information and referral, and advocacy surrounding health issues and insurance questions. The Part Time Care Manager for The Big Island will guide and monitor patients and families through the progression of the illness, refer patients and families to community services, facilitate monthly support groups, serve as liaisons to ALS Clinics, and assist with the implementation of treatment recommendations made by clinic staff. Under the direction of ALS Network leadership, they also represent the Organization, as needed, in statewide initiatives related to mission priorities of care services, advocacy, and research.
ESSENTIAL JOB RESPONSIBILITIES
Monitor and assist ALS patients, families, and caregivers on The Big Island in alignment with organizational protocols and productivity expectations. Assess psychosocial needs of patients, families, and caregivers. Provide appropriate counsel, information, and referrals including assistance with: Adjustment to diagnosis. Coping with the challenges of the disease. Managing the transitions of the illness. Ongoing guidance and psychosocial support to patients and families via phone, texting, email, Zoom or in-home visits. Facilitate equipment acquisition to assist with mobility, safety, respiration and communication through insurance or equipment loan program in alignment with ALS Network protocols. Responsible for coordinating the timely completion and submission of grant-related and ALS Network expense reimbursement paperwork Collaborate closely with the Senior Director of Care Services for approval of equipment and direct assistance expenditures on behalf of clients. Other administrative duties as assigned. Supervision, oversight and administration Direct supervision provided by Sr. Director of Care Services Work directly with equipment loan program staff to ensure that equipment needs are met on The Big Island and are in alignment with ALS Network protocols. Responsible for a timely and complete employee timecard. Responsible for coordinating the timely completion and submission of grant-related and ALS Network expense reimbursement paperwork. Work closely with the Director of Care Services for approval of equipment and direct assistance expenditures on behalf of clients Provide information, referrals and access to equipment and services Provide referrals to ALS Certified Centers and Clinics
  • Inform patients and families about ALS Network services.
Serve as an advocate for patients and families. Facilitate or arrange/oversee support groups. Provide information and referrals to health, community, and financial services. Provide referrals to local home health agencies and other vendors for services such as respiratory, social work or occupational therapy Serve as an ALS Network and client liaison to the healthcare, research, and advocacy-related community Represent the ALS Network at ALS clinics, providing information regarding ALS Network services & events and assisting people with ALS implementing clinic recommendations at home and in the community. Establish and foster strategic community and professional partnerships and collaborations, including designing, planning, and delivering relevant training. Maintain relationships with local neurologists and other providers through outreach activities. In collaboration with ALS Network Leadership, represent the ALS Network in Hawaii-based initiatives connected to local research and advocacy.
DESIRED BACKGROUND/SKILLS/KNOWLEDGE
LVN,RN, PT, OT, ST, RT OR Social Worker experience Currently reside in the state of Hawaii, on The Big Island, and familiar with local resources. A bachelor's degree in healthcare discipline, or at least 5-7 years of experience working with individuals living with significant disability or impairment. Excellent attention to detail, sensitivity to disability issues, timely data tracking, and reporting. Must possess the ability to work independently from home and commute to local area hospital-based clinics and client's homes.
WORKING CONDITIONS
This position is hybrid, home based position with attendance at local multi-disciplinary clinic, with an occasional evening and weekend hours for events and meetings. Direct service to ALS patients, families and caregivers can be provided by phone, email, virtual meeting platforms, and in-person visits to patients' homes
PHYSICAL REQUIREMENTS
Moderate and occasional lifting, pushing, pulling or exerting force of up to 25 pounds. Regular sitting and standing associated with a normal office environment. Regular reaching with hands and arms. Frequent and repetitive manual dexterity needed for using a cellphone, tablet, and computer keyboard. Occasional need to stoop, kneel, or crouch.
TRAVEL:
Valid Driver's License. Must have reliable transportation. Occasionally, local, regional, and limited national travel are required.
