Job Summary:
Champion Health Plan is seeking experienced LVN Case Managers to provide care management and conduct home visits. In this role, you will play a critical part in ensuring our Medicare
ESRD SNP
members receive the highest level of care and support. Full-time positions are available in California (Bakersfield, Kern County). Bilingual Spanish/English is preferred.
Key Responsibilities:

Care Coordination:
Collaborate with healthcare teams to coordinate and manage the care of Medicare members, ensuring that they receive timely and appropriate care services. •
Assessment and Planning:
Conduct comprehensive assessments of members' health needs and develop individualized care plans that address their unique requirements and goals. •
Education and Support:
Provide education and support to members and their families on managing chronic kidney disease, treatment options, and self-care strategies. •
Monitoring and Evaluation:
Continuously monitor members' health status and treatment progress, making necessary adjustments to care plans and interventions as needed. •
Advocacy:
Serve as an advocate for members, ensuring that they have access to the necessary resources and services to optimize their health outcomes. •
Documentation:
Maintain accurate and up-to-date electronic health records (EHRs) and documentation of member interactions, care plans, and interventions. •
Collaboration:
Work closely with healthcare providers, social workers, and other healthcare professionals to ensure seamless coordination of care. •
Travel:
Willingness to travel as needed to visit members, attend meetings, and collaborate with healthcare partners (travel expenses covered).
Qualifications:

Education:
Must have current LVN license. •
Bilingual Spanish/ English:
preferred. •
Experience:
Minimum of 2 years of experience in nursing, with home visit, case management experience. •
Communication:
Excellent communication and interpersonal skills to effectively engage with members, families, and healthcare teams. •
Technology:
Proficiency in electronic health record systems and Microsoft Office suite. •
Team Player:
Ability to work collaboratively in a multidisciplinary team and adapt to a fast-paced healthcare environment. •
Travel:
Must be willing to travel as required.
