We envision a community where people live and age in-place with health and dignity. We are seeking a Nurse Care Manager to support our mission.Position Summary:
As a Nurse Care Manager for Health Projects Center's Multipurpose Senior Services Program (MSSP), you play a pivotal role in delivering exceptional community-based care to frail, low-income senior participants in Santa Cruz, Monterey, San Benito, and San Luis Obispo Counties. Reporting to a collaborative and supportive team, you conduct comprehensive home health assessments, evaluating health, psychosocial, rehabilitation, and environmental factors to create holistic participant profiles. Your responsibilities include developing individualized care plans addressing various needs, coordinating local, state, and federal resources, and monitoring participant progress. Engaging in interdisciplinary team collaboration, you actively contribute insights and foster effective communication to ensure comprehensive participant care. With a focus on health education, medication management, electronic case recording, and participant advocacy, you serve as a dedicated advocate, liaising with healthcare providers and engaging with diverse communities. This position aligns seamlessly with Health Projects Center's commitment to enabling elders to live and age with dignity at home, and we welcome an enthusiastic and compassionate professional to join our team. Travel between counties is required.Essential Functions:
Comprehensive Home Health Assessments Conduct thorough home health assessments, delving into health, psychosocial, rehabilitation, and environmental concerns. Evaluate participants' individual needs and circumstances to create a holistic understanding of their health and living conditions. Care Plan Development Assess the level of care required based on individual participant needs. Develop detailed, individualized care plans addressing emergency services, nutritional needs, healthcare, medical attention, health education, emergency and permanent housing, mental health and substance abuse counseling, as well as emotional and practical support. Coordination of Services Coordinate participant needs for local, state, and federal benefits eligibility and other local resources. Provide direct provision, education, and referrals to ensure participants have access to necessary services and support. Progress Monitoring Regularly monitor and evaluate participant progress as outlined in their individualized care plan. Adjust care plans as needed based on ongoing assessments and participant developments. Medication Management Assist participants in managing medications, ensuring adherence to prescribed regimens and addressing any concerns or challenges. Interdisciplinary Team Collaboration Actively participate in interdisciplinary team meetings to share insights, updates, and collaborate on participant care strategies. Foster effective communication and teamwork among various professionals involved in participant care. Electronic Case Recording Utilize an electronic case recording system to provide timely and relevant chart notes, maintaining accurate and up-to-date participant records. Participant Advocacy Act as a dedicated advocate for participants, liaising with health care providers to ensure participants receive the best possible care and support. Travel and Community Engagement Travel between Santa Cruz, Monterey, San Benito, and San Luis Obispo Counties to meet participants where they live. Demonstrate a willingness to engage with diverse communities, including those of color, the LGBTQ community, and individuals with HIV disease. Requirements Possession of a valid and current license (RN) to practice nursing in the State of California, ensuring compliance with regulatory requirements. A minimum of two years of practical experience as a Registered Nurse, including at least one year in the field of community nursing. Demonstrated expertise in providing comprehensive healthcare services. Display a positive and constructive attitude towards participants, the organization, and its staff. Foster a collaborative and supportive work environment. Ability to travel within Santa Cruz, Monterey, San Benito, and San Luis Obispo Counties to fulfill job responsibilities effectively. Proven capacity to work effectively with diverse communities, including communities of color, the LGBTQ community, and individuals living with HIV disease. Demonstrate cultural sensitivity and inclusivity. Willingness to meet participants where they live, recognizing the potential challenges post-COVID. This may involve navigating unsanitary conditions and entering high-crime communities, emphasizing a commitment to serving those in need. Fluency in both English and Spanish preferred. Bilingual proficiency enhances the ability to communicate effectively with a diverse participant base.Note:
