BronxAbout the RoleWe are seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs.This role combines limited field work with remote care coordination, offering strong work-life balance while making a meaningful impact in the community.
What You'll DoConduct in-home assessments to evaluate members' clinical, functional, and social needsDevelop and manage individualized care plans in collaboration with physicians and interdisciplinary teamsCoordinate care across home care, hospital, rehabilitation, and long-term care settingsComplete approximately 6 home visits per week within BrooklynMonitor patient progress and adjust care plans to ensure optimal outcomesSupport safe discharge planning and reduce hospital readmissionsCommunicate with members, families, caregivers, and providersMaintain accurate and timely documentation in compliance with guidelinesQualificationsActive, unrestricted RN license in New YorkAssociate's degree in nursing required (BSN preferred)Minimum 3 years of RN experienceExperience in MLTC, home care, geriatrics, or case management preferredStrong communication, organizational, and critical thinking skillsAbility to work independently in a hybrid environmentBilingual skills are a plus
