Registered Nurse (RN) Care Manager Atlanta Structured Family Services If you have a passion for working with the elderly and individuals with disabilities and are seeking a flexible, hybrid work schedule, Atlanta Structured Family Services is the place for you. We are seeking a highly motivated Registered Nurse (RN) Care Manager to join our team. This role provides clinical oversight and supervision to LPNs while managing care for clients in a Home & Community-Based Services setting. Position Overview As an RN Care Manager, you are responsible for providing clinical oversight, guidance, and support to family caregivers enrolled in the
CCSP/SOURCE/EDWP
Structured Family Caregiving (SFC) program. The RN Care Manager supervises LPN staff, conducts comprehensive assessments, develops care plans, and ensures high-quality, compliant care delivery. This role works collaboratively with caregivers, care coordinators, and community partners to support client independence and well-being. Key Responsibilities Conduct in-home visits to assess client safety, health status, and caregiver needs Complete comprehensive clinical and psychosocial assessments Develop, implement, and oversee individualized care plans Provide clinical supervision and guidance to LPN staff Review and monitor documentation for accuracy and compliance Provide ongoing coaching, training, and support to caregivers and LPNs Maintain communication with caregivers, care coordinators, and internal staff Conduct supervisory visits and participate in case conferences Submit required reports (monthly, quarterly, and as needed) Ensure compliance with Medicaid waiver and regulatory requirements Build partnerships with community organizations and represent the agency in meetings Perform other duties as assigned Qualifications Active Registered Nurse (RN) license in the state of Georgia required Minimum 2 years of experience in home health, elder care, or related field Experience in care planning, assessments, and case management Supervisory experience preferred Strong communication, organizational, and leadership skills Ability to work independently in a flexible, field-based role Knowledge of community resources and Medicaid waiver programs preferred Schedule & Work Environment Flexible hybrid role (remote + in-home visits) Travel required within assigned service area

$45.00 - $50.00 per hour
