PLEASE NOTE
•All candidates must have a NY RN license and registration and a permanent residency in NY, NJ or CT. We are seeking a dedicated and highly skilled Registered Nurse Care Manager to join our healthcare team. This vital role involves coordinating comprehensive patient care, managing complex medical cases, and ensuring seamless communication across multidisciplinary teams. The ideal candidate will possess extensive clinical expertise, particularly in acute and post-acute care settings, with a focus on delivering exceptional patient-centered services. As a Registered Nurse Care Manager, you will play a critical role in optimizing patient outcomes through effective assessment, discharge planning, and utilization management. Duties Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to develop individualized care plans. Coordinate inpatient and outpatient care services across hospital, clinic, and post-acute settings to ensure continuity of care. Manage discharge planning processes by collaborating with physicians, case managers, and families to facilitate safe transitions from hospital to home or other facilities. Oversee utilization management activities to ensure appropriate resource use while maintaining compliance with healthcare policies such as HIPAA. Perform diagnostic evaluations and interpret medical records to support clinical decision-making and treatment adjustments. Provide expert nursing care in specialized areas such asICU, PICU
(Pediatric Intensive Care Unit), Geriatrics, Behavioral Health, or Infectious Disease care as needed. Engage in patient service initiatives by educating patients on health management strategies and promoting adherence to treatment plans. Document all clinical interactions accurately within electronic medical records systems while adhering to confidentiality standards. Qualifications at least 1 year of experience as a Care Manager an active RN license in NY State Ability to speak English and at least one of these languages (Russian, Cantonese, Mandarin, Bengali or Korean) Permanent residence inNew York State, New Jersey or Connecticut Pay:
$70,000.00 - $75,000.00 per yearBenefits:
