  • Full-time The Team Manager evaluates and coordinates the various members of the interdisciplinary team to assure continuity of care from admission to discharge.
Professional Duties Manages patient care services of the team to ensure services are rendered reflective of the plan of care. Coordinates palliative care measures for pain and symptom management with both the Attending Physician and Medical Director regarding palliative care measures for pain and symptom management. Assures complete and accurate data and documentation are available to make re-certification decisions and further assures that those decisions are made on a timely basis. Reviews all prospective potential live discharges for extended prognosis; to assure effective discharge planning. (Follow FHC protocol). Reviews the type and level of team services provided (staffing, medications, DME, supplies, level of care changes to inpatient and continuous care, etc.) to assure that they are appropriate and responsive to patient/family needs and expectations. Facilitates team meeting, assuring interdisciplinary input into the update and revision of the plan of care in concert with the changing needs and expectations of the patient and family. Participates in Administrative On-Call rotation. In conjunction with the Administrator/DPCS, responsibilities include: Hiring Interviews, selection, training, supervision, evaluation and quality assurance and performance improvement of team members. Oversees all team schedules to assure that workload is distributed equitably and that all staff are able to meet overall productivity expectations. Participates in Quality Assessment Performance Improvement. Participates in Community Outreach. Registered Nurse shall have a minimum of 1 year experience as a professional nurse within the last 3 years. A minimum of 3 years hospice supervisory or management experience preferred. A clear understanding of the hospice philosophy. Knowledge of the general principles and practices of nursing care. Excellent understanding of the Interdisciplinary Team process. Demonstrates the ability to supervise and direct professional and administrative personnel. Able to multi-task effectively. Effective listing and communication skills. At the discretion of
