Compensation Salary starting at: $73,500
  • $98,452/year based on experience.

We are seeking a seasoned MDS Program Manager to join our dynamic team! The MDS Program Manager has a huge impact on the lives of residents and on the overall culture of the community. This position is an invaluable part of the Still Hopes family! Below is a picture of the impact the MDS Program Coordinator has and what being a difference maker looks like! This position plays a vital role in the care planning process by ensuring timely and accurate completion of the Minimum Data Set (MDS) assessments. This position works collaboratively with interdisciplinary teams to develop comprehensive care plans that reflect the individualized needs of each resident. Coordinate and oversee the MDS assessment and care planning processes. Ensure accurate and timely completion of MDS assessments in accordance with federal and state guidelines. Collaborate with interdisciplinary teams to gather assessment data and ensure comprehensive resident care plans. Monitor Quality Measures and implement improvement strategies. Educate staff on MDS documentation requirements and support compliance. Participate in audits, surveys, and performance improvement initiatives. Must possess, as a minimum, a BS Degree from an approved school. Must possess nursing license for RN and be eligible to practice as an RN in the state of SC. Current Health Care Provider CPR Certification. Must have, as a minimum, three (3) years experience in a hospital, nursing care facility, or other related medical facility. Driver's license required with verified three (3) year safe driving record.
