Our mission is to enhance the quality of life for our patients through compassionate care and professional support. Summary We are seeking a Registered Nurse - Home Health to join our team. In this role, you will work in office and at times provide essential nursing care to patients in their homes, ensuring they receive the highest level of medical attention and support. Your contributions will be vital in promoting patient health and well-being. Responsibilities Conduct comprehensive patient assessments to determine care needs. Administer medications and treatments as prescribed by physicians. Monitor vital signs and document changes in patient condition. Maintain accurate medical records in compliance with HIPAA regulations. Provide education and support to patients and families regarding health management. Collaborate with interdisciplinary teams to develop and implement care plans. Ensure a safe and clean environment for patient care. Requirements Current Registered Nurse (RN) license in the state of practice. Experience in home health care or similar settings preferred. Strong knowledge of HIPAA regulations and patient confidentiality. Excellent assessment, critical thinking, and communication skills. Ability to work independently and manage time effectively. Key Responsibilities Clinical Oversight Review and approve patient assessments, plans of care, and clinical documentation. Ensure physician orders are received, signed, and implemented. Monitor patient progress and outcomes. Assist with clinical problem-solving and patient care concerns. Compliance & Quality Assurance Ensure compliance with state, federal, and agency regulations. Review charts for completeness and accuracy. Participate in audits and quality improvement initiatives. Maintain adherence to Medicare, Medicaid, and accrediting body requirements. Care Coordination Coordinate services among physicians, therapists, aides, and other disciplines. Manage referrals, admissions, recertifications, and discharges. Communicate changes in patient condition to appropriate parties. Ensure timely scheduling of visits and services. Staff Support & Supervision Provide clinical guidance to nurses, aides, and other field staff. Assist with orientation and training of new employees. Monitor competency and performance as assigned. Serve as a resource for clinical questions. Documentation Management Review OASIS assessments (if applicable). Ensure documentation supports services provided and reimbursement requirements. Track and follow up on unsigned orders and missing documentation. Patient & Family Support Address patient and family concerns that arise by phone. Assist with care planning and education. Help resolve service issues and complaints. Administrative Duties Participate in case conferences and interdisciplinary meetings. Assist with policy and procedure implementation. Maintain clinical records and reports. Support agency preparedness for surveys and audits.
