Job Description

Sage Oak of Denton is seeking a compassionate, dependable, and experienced Charge Nurse (LVN/RN) to join our Assisted Living and Memory Care team. We are looking for a strong clinical leader who is passionate about resident care, teamwork, and creating a positive environment for both residents and staff.
Position Details:
12-hour shifts (6 AM - 6 PM and 6PM-6AM) Rotating schedule Full-Time and PRN positions available
Location:
Sage Oak of Denton In-person position
Responsibilities:
Supervise and support the caregiving team during shifts Provide hands-on nursing care with compassion and professionalism Monitor resident conditions, administer medications, and coordinate care plans Collaborate with staff, families, physicians, and leadership to ensure exceptional resident care Maintain compliance with state regulations, safety standards, and company policies Foster a positive, resident-centered environment and strong team culture
Qualifications:
Current and valid Texas LVN or RN license required Minimum of 2-3 years of experience in Assisted Living and/or Memory Care required Strong clinical assessment, communication, and leadership skills Experience with medication administration and resident documentation Ability to work independently while leading and supporting the team Compassionate, reliable, and team-oriented attitude

Memory care: 2 years (Preferred)
License/Certification:
