Job Summary As the RN Director of Care Management, you will lead the strategic implementation and management of Care Management and Resource Management programs at Lake Granbury Medical Center. You will ensure regulatory compliance, optimize discharge planning, and mentor a high-performing team to deliver patient-centered outcomes in our community. Essential Functions Oversees the implementation and maintenance of an effective and compliant Care Management program. Provides leadership and direction to staff, ensuring accountability and adherence to scope of practice. Conducts chart reviews to evaluate severity of illness (SI) and intensity of services (IS). Refers complex cases to the Physician Advisor or Utilization Management Committee. Identifies avoidable days and implements communication systems to resolve them. Coordinates discharge planning and social services in compliance with federal/state laws and Joint Commission standards. Acts as a liaison to community agencies for post-hospital placement. Ensures accurate and timely reporting of CPS/APS cases. Participates in Patient Care Conferences and Length of Stay (LOS) meetings.Leadership & Strategic Oversight Staff Management :
Provide mentorship, conduct performance evaluations, and manage departmental scheduling.Financial Oversight :
Collaborate on budgeting, resource allocation, and cost-effective service delivery.Quality & Compliance :
Oversee accreditation processes and ensure all services meet or exceed industry standards. Develop and implement departmental policies, procedures, and protocols.Collaboration :
Work closely with hospital leadership to coordinate and improve service delivery. Stay up-to-date with industry advancements, new technologies, and regulatory changes.Qualifications Education :
Bachelor's Degree in a relevant field (or 7+ years of direct experience in lieu of degree). Master's Degree preferred.Experience :
3-5 years of experience in a closely related field (with Bachelor's).Leadership :
Previous leadership experience in a healthcare setting is required.Licensure :
Active RN License (Texas State or Compact State) required.Certification :
BCLS required within 7 days of hire.Benefits:
