What You'll Do As a Clinical Care Manager, you ensure coordination, continuity, and quality of care across your patient population. Coordinate care across clinicians, providers, and caregivers to ensure seamless delivery Support referrals and admissions, helping ensure patients are seen in a timely manner Manage and process physician orders, ensuring accuracy and follow-through Oversee development and updates to individualized care plans Monitor patient progress and ensure reassessments occur when conditions change Conduct in-home visits as needed to support patient care and team alignment Review clinical reports (occurrences, infection control, on-call notes) and take action Communicate effectively with physicians and interdisciplinary teams Participate in case conferences and support care decisions (recertifications, discharges) Identify gaps or risks and proactively address or escalate What You Bring Active RN license in the state of Texas Graduate of an accredited nursing program Current CPR certification Strong organization, communication, and critical thinking skills Ability to manage multiple priorities and follow through consistently Preferred (not required): Experience in home health, case management, or care coordination Previous leadership or supervisory experience BSN Our Values We lead with: Trustworthy.
