Job Description

Job Summary We are seeking a dedicated and skilled Licensed Vocational Nurse (LVN) to join our healthcare team. The LVN will provide compassionate patient care across various settings, including acute, long-term, and community-based care environments. The ideal candidate will possess extensive clinical experience, geriatrics, and working with diverse patient populations such as individuals with developmental disabilities, dementia, and those requiring hospice or palliative care. This role requires proficiency in medical documentation, Point Click Care (PCC) , and a strong understanding of medical terminology and physiology. The LVN will collaborate with interdisciplinary teams to ensure high-quality patient outcomes and adherence to healthcare standards. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and intake evaluations. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and aseptic techniques. Perform IV infusions, catheterizations, venipuncture, and other invasive procedures with precision and safety. Manage airway support and airway management techniques. Document all patient interactions thoroughly in EMR/EHR systems such as Point Click Care (PCC). Assist with diagnostic evaluations such as laboratory specimen collection/processing, medical imaging procedures, and spinal taps. Provide specialized care for patients with complex needs such as Alzheimer's/dementia care, pain management, post-acute care, hospice services, and behavioral health interventions. Support discharge planning and case management activities to ensure seamless transition of care across settings. Educate patients and families on health coaching topics including nutrition, medication administration, wound care (sutures), and disease management strategies. Collaborate with multidisciplinary teams in various clinical environments such as ICU, emergency room, outpatient clinics, home & community care settings, neonatal units (neonatal ventilator), pediatric units (pediatric ICU), dialysis centers, and occupational health clinics. Qualifications Valid LVN license in the state of practice. Proven experience in diverse clinical settings such as medical-surgical units, geriatrics (long-term & assisted living), hospice or palliative care. Proficiency in EMR/EHR systems including Point Click Care (PCC). Strong knowledge of medical terminology, anatomy & physiology, pharmacology (including medication administration & injection techniques), infection control protocols, and aseptic techniques. Experience working with individuals with disabilities or developmental delays is a plus. Ability to perform complex procedures such as gastric lavage, dialysis support, sterile processing skills. Excellent patient assessment skills along with the ability to work effectively within multidisciplinary teams in fast-paced environments such as hospitals or outpatient clinics. Must pass background checks and drug screenings prior to employment; current Basic Life Support (BLS) certification required. Pay:
$26.00 - $28.00 per hour
