Home Health Job Summary We are seeking an experienced in-office RN Case Manager to play a key leadership role in ensuring high-quality, patient-centered care across the home health continuum. This position is responsible for coordinating care through established Plans of Care and physician orders while optimizing resource utilization, regulatory compliance, and clinical outcomes. The RN Case Manager serves as a clinical resource, patient advocate, and operational leader across assigned regions through in-person, remote, and telephonic care coordination. Key Responsibilities Collect and review SOC, Recertification, Evals and Re-Eval reports from clinicians Develop, review, and modify Plans of Care and physician orders to ensure clinical appropriateness and regulatory compliance Track hospitalizations, falls, infections, and incident reports to ensure appropriate follow-up and reporting Advocate for patients by delivering care that is safe, timely, effective, efficient, equitable, and patient-centered Provide supervision and clinical guidance to staff to ensure compliance with Plans of Care and continuity of services Collaborate closely with nursing, therapy, and interdisciplinary team members Communicate effectively and in a timely manner Serve as the primary clinical point of contact for assigned patients Communicate patient status and care needs to internal teams as deemed Provide clinical guidance and support to field staff as needed Participate in case conferences and interdisciplinary meetings Assess readiness for discharge Ensure goals are met or appropriately transitioned Provide final education and resources Coordinate post-discharge services as needed Participate in staff education and mentoring Assist with on-call coverage as assigned Assist leadership with clinical workflow improvements Perform additional duties and responsibilities as assigned by the DON, ADON, or Director of Case Management Required Experience Minimum of two (2) years of RN Case Management experience 1 year of home health RN Case Management experience Proficiency in data analysis with strong verbal and written communication skills Skilled in patient assessment techniques including vital signs measurement, physical examinations, phlebotomy, wound vacs, venipuncture, and specimen collection/processing. Experience working with diverse populations including pediatrics , geriatrics or those with developmental disabilities. Licensure & Requirements Active Texas RN license or Nurse Licensure Compact (NLC) license Valid Texas Driver's License Reliable transportation with current auto liability insurance Authorized to work in the United States Work Environment Office-based during agency operating hours ( Monday-Friday, 8:30 AM
