Lead PCM Position Summary The Patient Care Manager (PCM) is a Registered Nurse who provides daily leadership and oversight of the hospice clinical team in partnership with the Director of Clinical Services. This role is central to ensuring that patients and families receive high-quality, timely, and compassionate hospice care. The PCM supports the clinical team, coordinates patient care activities, oversees documentation, and fosters a culture of collaboration and excellence. Key Responsibilities Leadership & Supervision
- Provide leadership and daily oversight for the hospice clinical team (RN Case Managers, LVNs, Hospice Aides, Social Workers, and Chaplains).
- Deliver mentorship, coaching, and performance feedback to promote accountability and professional growth.
- Support orientation and training for new clinical staff to ensure consistency and excellence. Hospice Care Coordination
- Oversee and coordinate patient care to ensure delivery matches the individualized plan of care.
- Participate in and help lead Interdisciplinary Group (IDG) meetings.
- Monitor hospice eligibility, re-certifications, and Face-to-Face encounter compliance. Compliance & Quality Assurance
- Ensure clinical documentation is accurate, timely, and compliant with hospice standards.
- Conduct chart audits and clinical reviews to monitor quality of care and identify opportunities for improvement.
- Assist with survey readiness and participate in quality improvement activities.
- Collaborate with the Director of Clinical Services to identify gaps in care or performance and implement solutions. Operational Support
- Manage staff scheduling and assignments to meet patient and family needs.
- Act as a clinical resource and liaison for referral partners, patients, and families.
- Other tasks as needed Qualifications
- Passion for excellence in hospice care
- Current Registered Nurse (RN) license in the State of Texas (required).
- Minimum 2 years of hospice nursing experience (required).
- Leadership, supervisory, or case management experience in hospice (required).
- Strong knowledge of hospice care standards and best practices.

Environmental and Working Conditions:
The position may be stressful at times, especially when meeting deadlines. Minimal lifting of office records, supplies, and printouts may be required. This is primarily a desk job that involves sitting, standing, stooping, walking, and frequent telephone communication. Employee must have the ability to communicate in English by phone, in writing, and verbally with various levels of staff, patient families, and external contacts, in accordance with the company's HIPAA policy. Bilingual is a plus.
