Job Overview We are seeking a dedicated Memory Care Charge Nurse/LVN to join our team during the 10pm-6am shift. In this vital role, you will oversee the daily care of residents with memory impairments, ensuring their safety, comfort, and dignity. You will coordinate clinical activities, support staff, and implement personalized care plans tailored to each resident's needs. Your expertise will help create a positive environment that promotes quality of life for our residents living with Alzheimer's disease and other forms of dementia. Responsibilities Lead and supervise caregivers during your shift, providing guidance and support in delivering exceptional memory care. Assess residents' physical and emotional needs through comprehensive patient assessments and ongoing monitoring of vital signs. Administer medications, injections, and other treatments following strict protocols to manage acute pain and other health conditions. Develop, update, and implement individualized care plans in collaboration with the interdisciplinary team to enhance residents' well-being. Ensure compliance with HIPAA regulations and maintain accurate medical documentation within electronic health record (EHR) systems. Monitor for signs of infection or other health issues, coordinating triage and urgent care responses as needed. Educate family members on care plans. Skills Proven experience in long-term care settings or assisted living environments with a focus on dementia or Alzheimer's care. Strong knowledge of medication administration protocols. Experience with EMR/EHR systems Skilled in patient assessment techniques including vital signs measurement and physical examinations. Ability to manage acute situations involving pain management, or infectious disease control effectively. Excellent communication skills for working with residents with developmental disabilities, families, and interdisciplinary teams.
