This full-time opportunity offers a competitive salary range of $39.32 - $68.95 per hour while working alongside a collaborative, patient-centered rehabilitation team. Requirements to Apply Master's Degree in Social Work with medical experience Training in psychosocial aspects of physical disabilities and illness Minimum 3 years of clinical experience Experience supporting patients with physical disabilities, chronic illness, or rehabilitation needs Job Overview As a Care Manager RN, you will coordinate inpatient rehabilitation stays from admission through discharge while partnering closely with physicians, therapists, nurses, patients, and caregivers. This role focuses on discharge planning, patient education, care coordination, and ensuring patients receive high-quality, cost-effective care tailored to their individual recovery goals. The ideal candidate is compassionate, organized, collaborative, and experienced in supporting patients navigating complex medical and rehabilitation needs.
