Registered Nurse (RN) - Named RN / Resident Care Manager/Ventilator Specialty AFH Vancouver, WA Full-Time | Benefits Eligible Join a Specialized Ventilator Care Team Ventilator Specialty Adult Family Home is seeking an experienced Registered Nurse to serve as our Named RN and Resident Care Manager (RCM) . This leadership position plays a critical role in ensuring exceptional resident care, regulatory compliance, and clinical oversight within our specialized ventilator and tracheostomy care home. This is an excellent opportunity for an RN with experience in long-term care, ventilator care, tracheostomy management, adult family homes, skilled nursing, or medically complex populations who is looking to make a meaningful impact in a smaller, resident-focused environment. Position ResponsibilitiesResident Care Oversight Review and update resident care plans every six months to ensure accuracy and compliance.
Review and reconcile:
Private Duty Nursing (PDN) logs State assessments Physician orders Medication lists Observation and progress notes Review and coordinate physician care plan updates every six months. Collaborate with providers to ensure resident needs and services remain current and appropriate. Ensure care plans, assessments, medication records, and physician orders accurately reflect resident needs. Clinical Leadership Provide clinical oversight and support to nursing staff and caregivers. Assist with admissions, discharges, and resident assessments. Support caregiver education, competency validation, and ongoing training. Promote quality improvement and regulatory compliance throughout the facility. Serve as a resource for families, healthcare providers, and staff. Regulatory Compliance Ensure compliance with Washington State Adult Family Home regulations. Maintain accurate resident records and documentation. Participate in state surveys, audits, and inspections. Serve as a clinical point of contact during licensing visits and investigations. Work collaboratively with ownership and facility leadership to maintain high standards of care. Qualifications Active Washington State Registered Nurse (RN) license in good standing. Experience caring for medically complex residents. Ventilator and tracheostomy care experience strongly preferred. Experience in long-term care, skilled nursing, rehabilitation, adult family homes, or home health preferred. Strong assessment, documentation, and care planning skills. Excellent communication and leadership abilities. Ability to work independently and collaboratively with interdisciplinary teams. Preferred Qualifications Adult Family Home experience. Nurse Delegation experience. Resident Care Manager experience. Experience working with ventilator-dependent and tracheostomy-dependent residents. Experience with electronic charting systems.
