Oakwood Creative Care is seeking an Occupational Therapist who is passionate about helping individuals maintain independence, purpose, and quality of life. This role goes beyond traditional therapy—blending clinical care with coaching, education, and real-world strategies that support both individuals and their families. You'll work closely with an interdisciplinary team to assess needs, develop care plans, and provide practical tools that enhance daily function and reduce challenges associated with cognitive change. What You'll Do Evaluate strengths and support independence in daily living Develop and implement personalized care plans Educate and coach family care partners Lead community workshops on brain health and aging What You Bring OT degree and Arizona license Experience with older adults or dementia preferred Strong, person-centered approach Passion for innovative, community-based care
