Full-Time Position Summary:
We are seeking a compassionate and skilled Occupational Therapist to provide home health services to patients throughout Tempe and the surrounding East Valley communities. The Occupational Therapist will assess patients' functional abilities and develop individualized treatment plans to improve independence with activities of daily living (ADLs), safety, and quality of life within the home environment.
Essential Responsibilities:
Conduct comprehensive occupational therapy evaluations in patients' homes. Develop and implement individualized treatment plans based on physician orders and patient needs. Assess and treat deficits related to activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognition, and home safety. Recommend adaptive equipment, assistive devices, and environmental modifications to maximize patient independence. Educate patients, family members, and caregivers regarding treatment plans and safety strategies. Collaborate with physicians, nurses, physical therapists, speech therapists, social workers, and other healthcare professionals. Maintain timely and accurate documentation in compliance with agency, state, and federal regulations. Participate in interdisciplinary team meetings and care planning.
Qualifications:
Graduate of an accredited Occupational Therapy program. Current Arizona Occupational Therapist license in good standing. Current CPR certification. Minimum one year of clinical occupational therapy experience preferred. Home health experience preferred but not required. Valid driver' s license, reliable transportation, and current automobile insurance. Strong communication, organizational, and documentation skills.

Monday Friday, daytime hours. Flexible scheduling available. Occasional weekend coverage may be required.
