Occupational Therapist, Home Health PRN Ascent Allied Solutions, LLC Citrus Heights, CA Job Details PRN | Per diem $100 - $120 an hour 21 hours ago Benefits Flexible schedule Qualifications Care documentation Patient interaction Home health Full Job Description Job Summary Overview At Ascent Allied Solutions , we are seeking a dedicated Occupational Therapist (OT) to join our expanding team in Citrus Heights / Sacramento Area . In this position, you will assist patients in regaining independence and enhancing their ability to carry out meaningful daily tasks. You will evaluate functional needs, create treatment plans, and implement evidence-based occupational therapy interventions for home health. Your efforts will play a crucial role in helping patients develop the necessary skills and confidence for everyday life, whether they are recovering from an injury, managing chronic illness, or adjusting to new physical or cognitive challenges. Duties Perform thorough evaluations to determine limitations and establish realistic goals Create and execute personalized care plans aimed at improving activities of daily living (ADLs), fine motor skills, coordination, and cognitive abilities Educate patients and caregivers on adaptive strategies, assistive devices, and home modifications to encourage safety and independence Work collaboratively with interdisciplinary teams to coordinate patient care and facilitate effective discharge planning Track patient progress and modify treatment plans as necessary to ensure the best possible outcomes Maintain precise and timely documentation using EMR systems, following healthcare regulations Skills and Qualifications Experience in creating patient-focused care plans for varied populations, including elderly individuals and adults with neurological or orthopedic issues. Familiarity with home health care and community-based environments. Capacity to assess and address cognitive, sensory, and motor impairments. Proficient in using EMR systems for documentation and tracking patient care. Strong communication skills with a compassionate, patient-first approach. Understanding of adaptive equipment and strategies for environmental modifications.
