Occupational Therapist:

Provide occupational therapy services in accordance with applicable law, physician orders, payer requirements, and the authorized plan of care. Evaluate patients in their homes and provide skilled occupational therapy interventions to support safety, independence, functional mobility, activities of daily living, and quality of life. Develop, update, and implement care plans consistent with clinical judgment, physician orders, payer requirements, and applicable professional standards. Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as clinically appropriate and legally permitted. Communicate with the referring agency, physician, patient, family, COTA, or other authorized care participants as clinically appropriate and legally permitted. Document patient progress accurately and efficiently in accordance with applicable payer, agency, and regulatory requirements.

What We're Looking For:
