Opens a new window. Occupational Therapist (Modesto, CA) (multiple positions). Coordinate and provide restorative and rehabilitative Occupational Therapy (OT) services to participants at the Center and at home. Work closely with PACE Physician, rehabilitation staff, and interdisciplinary team (IDT) members to maximize participant independence, safety, and enhance performance of ADLs. Conduct OT assessments to determine care needs, preferences and goals of the participants. Participate in IDT meetings to develop participant care plans. Deliver and document OT interventions in accordance with physician orders or as agreed upon in the participants' care plans. Use devices, techniques, and activities to promote muscle re-education, perceptual-motor, and cognitive function, balance, endurance, and other factors affecting the level of independence in performing ADLs. Work with members of the IDT to support smooth care transitions between settings (hospitals, skilled nursing facilities, home). Participate in end-of-life care coordination and support. Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families. Complete all mandated occupational health screenings as needed. Local travel required to patient sites within normal commuting distance of Modesto, CA for mobile visits.

Salary:

$126,484 to $141,000 per year. Requires a Bachelor's degree or foreign equivalent in Occupational Therapy or a related field, and five (5) years (60 months) of experience in the job offered or in a related position providing Occupational Therapy services for PACE-eligible seniors. Also requires two (2) years of experience in each: A) delivering occupational therapy to patients to regain function; B) planning patient discharges with an interdisciplinary team; C) using CRM software to manage, track, and store patient data; D) providing durable medical equipment recommendations; and E) auditing occupational therapy care plans.
