This position offers flexible scheduling, 1:1 patient care, and strong clinical support from an experienced leadership team. Responsibilities Perform in-home occupational therapy evaluations and treatments Assess patient functional abilities, including strength, coordination, endurance, communication, and ADLs Evaluate home environments for safety risks and barriers to independence Develop, implement, and update OT plans of care in collaboration with RNs and physicians Complete OASIS documentation, progress notes, and re-evaluations in a timely manner Supervise Occupational Therapy Assistants (OTAs) as assigned Monitor patient progress and report significant changes to the care team Obtain physician orders to modify plans of care Educate patients and caregivers on adaptive equipment, safety strategies, and independent living techniques Qualifications Active Occupational Therapist (OT) license
