Our interdisciplinary team works collaboratively to support participants' physical, cognitive, social, and emotional well-being in a safe and engaging environment. Position Summary We are seeking a licensed Occupational Therapist to join our Adult Day Health Care team. The Occupational Therapist is responsible for evaluating participants, developing individualized treatment plans, and providing therapeutic interventions that promote independence, safety, and overall quality of life. The ideal candidate is compassionate, organized, and committed to helping older adults maintain and improve their functional abilities through evidence-based occupational therapy services. Key Responsibilities Conduct comprehensive occupational therapy assessments to identify participant needs, strengths, and goals Develop and implement individualized Occupational Therapy Plans of Care Provide therapeutic interventions to improve activities of daily living (ADLs) and functional independence Monitor participant progress and modify treatment plans as needed Educate participants and families on adaptive techniques, therapeutic exercises, and assistive equipment Recommend adaptive equipment and environmental modifications to improve safety and independence Collaborate with nurses, social workers, rehabilitation staff, and other interdisciplinary team members Maintain accurate and timely documentation, including assessments, treatment plans, and progress notes Participate in care conferences, team meetings, and treatment planning activities Ensure compliance with all applicable clinical standards, program regulations, and documentation requirements Perform additional duties as assigned to support participant care and program operations Qualifications Required Graduate of an accredited Occupational Therapy program Current and active Occupational Therapist license in the State of California Strong clinical assessment and treatment planning skills Excellent communication and interpersonal abilities Strong organizational, documentation, and time management skills Preferred Experience working with older adults, seniors, or individuals with disabilities Experience in adult day health care, rehabilitation, geriatric care, or community-based healthcare settings Knowledge, Skills & Abilities Knowledge of occupational therapy principles, rehabilitation practices, and functional assessment techniques Ability to develop and implement individualized treatment plans Ability to work collaboratively within an interdisciplinary healthcare team Ability to educate participants and families effectively Commitment to participant-centered care, dignity, and respect Ability to work independently while maintaining strong communication with team members Why Join Us?
