Job Description

OCCUPATIONAL THERAPIST
•FULL TIME•THE
SPRINGS AT ST ANDREWS VILLAGE
•AURORA, CO Key Responsibilities Screen and evaluate patients with cognitive and/or ADL‑related functional limitations; develop and implement individualized plans of care in accordance with regulatory and clinical standards Utilize standardized assessments and evidence‑based interventions to support patient outcomes Provide skilled, medically necessary occupational therapy services in an outpatient setting Deliver interventions including (but not limited to): ADL retraining, therapeutic activities and exercises, modalities, and use of adaptive or assistive equipment Monitor patient progress and modify treatment plans as clinically indicated; document accurately in the electronic medical record Evaluate treatment effectiveness and collaborate on referrals or care adjustments as needed Communicate effectively with the interdisciplinary team regarding patient progress, barriers, and goals Provide education, consultation, and guidance to patients, families, caregivers, and team members Complete discharge planning and summaries in compliance with clinical and regulatory requirements Accurately track treatment minutes and maintain documentation standards Participate in team meetings, care conferences, and trainings Adhere to NBCOT, AOTA, state practice acts, and company policies, including ethics and compliance standards Supervise or support COTAs, aides, or students as assigned based on availability and role needs Qualifications Active Colorado Occupational Therapist license Must be a graduate of an accredited program in Occupational Therapy New graduates and experienced clinicians welcome Equal Opportunity Employer.
