In this vital role, you will assess, plan, and implement personalized therapy programs across diverse settings, including inpatient units, outpatient clinics, schools, home health, skilled nursing facilities, and post-acute care environments. Duties Conduct comprehensive assessments of patients' physical and cognitive abilities to develop tailored care plans that promote functional independence. Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and mobility to enhance their autonomy. Collaborate with multidisciplinary teams—including speech therapists, physicians, nurses, and educators—to coordinate effective treatment strategies. Manage discharge planning by evaluating patient progress and recommending appropriate post-treatment services or home modifications. Provide therapy interventions in various settings such as inpatient units (including ICU experience), outpatient clinics, schools (early intervention and school experience), and home health visits. Support patients with diverse needs—including those with autism spectrum disorder, disabilities, geriatrics concerns, or recovering from acute or post-acute conditions—by designing specialized therapy programs. Maintain detailed documentation of patient progress, care plans, and case management records in compliance with healthcare regulations. Stay current with physiology knowledge, medical terminology, anatomy insights, and speech therapy techniques to deliver evidence-based care. Qualifications Bachelor's or Master's degree in Occupational Therapy from an accredited program; licensure or certification as required by state law. Proven experience working with people with disabilities across various settings such as skilled nursing facilities, inpatient units (including ICU), outpatient clinics, schools (early intervention), and home health environments. Strong understanding of medical terminology, anatomy, physiology principles relevant to occupational therapy practice. Experience assisting with ADLs and developing individualized care plans for diverse populations including pediatrics and geriatrics. Ability to collaborate effectively within multidisciplinary teams and communicate clearly with patients and families. Knowledge of case management practices and discharge planning procedures. Familiarity with working in acute care settings or outpatient clinics is highly desirable; ICU experience is a plus.
