Choose your own hours and make a difference in patients' lives! We offer offering skilled nursing, rehabilitation and non-skilled services for the residents in the community that require support with ADL's that may have a physical and/ or developmental disability. As a Occupational Therapist on our team, you will assess patients, create treatment plans, provide exercises, track progress, and educate on injury prevention while collaborating with the healthcare team.

Licensed Occupational Therapist in Connecticut Reliable transportation, and a willingness to cover Norwalk and surrounding areas Proven experience in Occupational therapy, preferably in a home health or clinical setting Strong understanding of musculoskeletal and neurological conditions Excellent communication and interpersonal skills Ability to develop and implement individualized treatment plans Knowledge of adaptive equipment and assistive devices
Responsibilities:
Evaluate patients' physical conditions and create personalized treatment plans Develop and implement therapeutic exercises to improve mobility, strength, and function Monitor patient progress and adjust treatment plans as needed Educate patients and their families on exercises, safety techniques, and home care Collaborate with healthcare teams to ensure comprehensive care Maintain accurate and up-to-date patient records Provide guidance on injury prevention and rehabilitation strategies
