Join our dedicated team as a Home Health Therapist (Physical Therapist, Occupational Therapist, Speech-Language Pathologist, PTA, or COTA) working PRN (as needed) or part-time! In this vital role, you will deliver personalized therapy services directly in patients' homes, empowering individuals to regain independence and improve their quality of life. This position offers flexibility, meaningful patient interactions, and the opportunity to make a real difference in your community. You'll work independently while collaborating with a supportive healthcare team to provide exceptional care tailored to each patient's unique needs. Responsibilities Assess patients' physical, occupational, or speech therapy needs through comprehensive evaluations in their home environment. Develop and implement customized treatment plans aligned with physicians' orders and best practices. Deliver therapy sessions that promote functional improvements and support patients' goals for independence. Educate patients and their families on therapeutic exercises, adaptive techniques, and safety measures for ongoing care. Monitor progress regularly and adjust treatment plans to maximize outcomes. Maintain detailed documentation of assessments, interventions, progress notes, and discharge summaries in compliance with regulatory standards. Communicate effectively with physicians, caregivers, and healthcare team members to coordinate care plans and ensure continuity of services. Qualifications Valid licensure or certification as a Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), Physical Therapist Assistant (PTA), or Certified Occupational Therapy Assistant (COTA). Prior experience in home health or outpatient therapy is preferred but not required; we value motivated learners eager to grow professionally. Strong interpersonal skills with the ability to motivate and educate patients of diverse backgrounds. Ability to work independently while managing a flexible schedule across different patient locations. Excellent organizational skills for documentation and compliance with healthcare regulations. Basic computer proficiency for electronic health records and communication tools. Pay:

$40.00 - $80.00 per hour
