Job Description

Occupational Therapist (OT) Lakes of Clermont Health and Rehabilitation Center is seeking a dedicated and enthusiastic Occupational Therapist (OT) to join our team in a full-time, per diem capacity. In this rewarding role, you will work directly with residents in a subacute rehabilitation setting, helping them regain independence and improve their daily living skills.
Key Responsibilities:
Patient Assessment:
Conduct comprehensive evaluations to assess patients' physical, cognitive, and emotional needs.
Customized Treatment Plans:
Develop and implement individualized treatment plans tailored to each resident's goals and rehabilitation needs.
Therapeutic Interventions:
Utilize a variety of therapeutic activities and techniques to enhance residents' functional abilities, mobility, and quality of life.
Patient and Family Education:
Provide education to patients and their families on therapeutic strategies, adaptive techniques, and home exercise programs to support recovery.
Collaboration:
Work closely with a multidisciplinary team, including physical therapists, speech-language pathologists, nurses, and physicians, to ensure coordinated care.
Progress Evaluation:
Regularly monitor and assess patient progress, adjusting treatment plans as necessary to optimize outcomes.
Documentation:
Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes, adhering to regulatory standards.
Qualifications:
Education:
Master's or Doctorate degree in Occupational Therapy from an accredited program.
Licensure:
Current state licensure or eligibility for licensure as an Occupational Therapist.
Experience:
Prior experience in a rehabilitation or healthcare setting is preferred, but new graduates are encouraged to apply.
