OT:

Patient Care:

Provide high-quality, one-on-one occupational therapy as a field OT to geriatric patients in their homes.

Care Planning:

Implement effective plans of care to improve patient mobility and safety.

Timely Documentation:

Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with a clinical team that helps you get it right.

Team Coordination:

Collaborate with our clinical leadership team, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care.

What you need:

License:

Active Florida Occupational Therapist (PT) License.

Certification:

Current CPR/BLS Certification.

Transportation:

Valid Driver's License and reliable transportation.

Software:

Kinnser/WellSky experience is a major plus! Actify Home Care has active OT home health cases ready for assignment across our expanding territory. Immediate cases available! Also, seeking coverage east of I-95 in Pompano Beach, Fort Lauderdale, Oakland Park and Dania Beach. Compensation Structure & Position Disclosures Earnings are paid strictly as flat fees on a per-completed-visit basis. This is a per-visit contract structure with no base salary, hourly base pay, or guaranteed minimum earnings. Flat rates for completed clinical visits range from $65.00 to $80.00 per visit depending on the case type, structured as follows: OASIS Visit): $80.00 flat rate per completed visit

Evaluation:

$70.00 flat rate per completed visit

Routine Patient Visit:

$65.00 flat rate per completed visit

1099 Independent Contractor (PRN / Per Diem)

Patient Homes (Home Health Field Visits)

Service Provided:

Occupational Therapy Assessments, Plan of Care Development, and Routine Therapeutic Interventions.

Guaranteed Hours:

None. This is an as-needed (PRN) contract role with flexible routing. Flexible Routing & Service Territory Because we know your time is valuable, your routing is designed around your preferred coverage zones. While our primary geographic pin is mapped to the posted zip code, this mobile field position offers the flexibility to select patient home cases within your comfort level throughout Coral Springs, Margate, Coconut Creek, and the surrounding residential areas.
