Evaluate, plan, and implement individualized therapy programs Collaborate with an interdisciplinary healthcare team Track/document patient progress and outcomes Educate patients and families on therapeutic strategies Stay current with innovative therapy techniques Assign patients to the team members Collaborate with Facility case management for Discharge planning and patient care management
