Responsibilities Must he willing to serve patients in Lee County, Collier County and the Islands. Conduct comprehensive patient assessments focusing on functional abilities among senior populations. Develop and implement individualized care plans utilizing evidence-based techniques tailored to each patient's goals. Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless transition across inpatient, outpatient, home health, and community settings. Manage patient care documentation accurately using electronic medical records (EMR) systems while adhering to healthcare regulations. Provide education to patients and their caregivers on injury prevention, activity modifications, and home exercise programs. Skills Strong knowledge of physiology, anatomy, medical terminology, and care plan development for diverse patient needs. Proven experience in inpatient hospital settings, outpatient clinics, home health environments, or skilled nursing facilities serving seniors. Expertise in discharge planning, acute care procedures, and home & community care coordination. Skilled in applying physiotherapy techniques such as trigger point therapy and orthopedics-focused interventions. Familiarity with EMR systems for efficient documentation and patient record management. Excellent patient care skills with a compassionate approach tailored to geriatric populations. Ability to work effectively within multidisciplinary teams while demonstrating strong communication skills.
