Join a clinical team led by clinicians, for clinicians. Actify Home Care is clinician-owned and operated by a Physical Therapist with over 20 years of experience. Territory:
We are now expanding our high-quality care throughout Palm Beach County, hiring skilled Geriatric Occupational Therapists to join our team. This position covers our North Palm Beach Zone , including Jupiter and Palm Beach Gardens. Why Therapists Choose Actify?

Direct deposit every two weeks.

We utilize Kinnser (WellSky) for streamlined charting.
What You'll Do:
Provide high-quality, one-on-one occupational therapy to geriatric patients in their homes. Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Implement plans of care to improve activities of daily living (ADLs) and patient safety. Compensation (1099 / PRN):
OASIS Rate:
$80.00 per visit Evaluation /
Treatment:

What You Need:
Active Florida Occupational Therapist (OT) License. Current CPR/BLS Certification. Valid Driver's License and reliable transportation. Kinnser/WellSky and OASIS experience is a plus.
