Position Summary :
The Occupational Therapist (OT)
  • Home Health is responsible for evaluating, planning, and implementing individualized therapy programs for patients in their homes to improve their functional independence and quality of life.
Job Duties :
Evaluate patients in their homes to assess physical, cognitive, psychosocial, and environmental needs. Develop and implement individualized treatment plans to improve patients' functional independence and quality of life. Provide therapeutic interventions to enhance activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
