In this vital role, you will provide comprehensive occupational therapy services to diverse patient populations across various settings, including inpatient units, outpatient clinics, schools, and community-based programs. Your expertise will help individuals of all ages regain independence, improve functional abilities, and enhance their quality of life. Responsibilities Assess patients' physical, cognitive, and emotional needs through detailed evaluations, including physiology and anatomy knowledge. Develop and implement individualized care plans tailored to each patient's goals and medical conditions. Assist with activities of daily living (ADLs) such as dressing, grooming, and mobility to promote independence. Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transitions from hospital to home or other care settings. Work with individuals with disabilities, autism spectrum disorder, geriatrics, pediatrics, or those recovering from injury or illness across inpatient, outpatient, skilled nursing facilities, home health, or school environments. Provide patient care that includes therapeutic exercises, adaptive equipment recommendations, and environmental modifications. Engage in case management activities to monitor progress and adjust treatment strategies accordingly. Support early intervention programs by working with children in school or community settings to foster developmental milestones. Contribute to the creation of detailed documentation such as progress notes and care plans while maintaining compliance with medical terminology standards. Educate patients and families on therapy techniques and strategies for ongoing support at home or in community settings. Experience Proven experience working in acute care environments such as ICU or inpatient units is highly desirable. Prior skilled nursing facility experience or post-acute care background is preferred. Demonstrated ability to work effectively with individuals with autism or other developmental disabilities in school or community settings. Experience providing occupational therapy services in outpatient clinics or home health environments is advantageous. Strong knowledge of physiology, anatomy, medical terminology, and patient care protocols is essential. Familiarity with speech therapy concepts can be a plus for interdisciplinary collaboration. Previous involvement in discharge planning, case management, and developing comprehensive care plans will set you apart.
